Disease Management Overview

St. Rita's Center for Disease Management is an approach to patient care that emphasizes coordinated, comprehensive care for those coping with chronic conditions such as asthma, heart failure or any condition requiring the use of medications that decrease the ability of the blood to clot. Careful management of these conditions is essential for patients to reduce their risk of complications and potential hospitalizations.

It is important for the patient to play an active role in the management of their health condition. St. Rita's professionals work with patients to develop an individualized treatment plan to help them keep their disease under control and to prevent frequent readmissions to the medical center. By working closely with patients, the Center for Disease Management can help them improve their compliance with medication, diet and exercise and assist in reducing any complications from their condition.

St. Rita's Center for Disease Management offers programs in:

  • Asthma Management
  • Congestive Heart Failure Management
  • Coumadin Management
  • Peripheral Vascular Disease Diagnosis and Treatment

The Center for Disease Management is staffed by a clinical pharmacist, registered nurses, clinical manager, respiratory therapist, licensed social worker and medical office assistants. Laboratory support on-site allows patients to have their blood draws completed at the Center without having to move to another location.

St. Rita's Center for Disease Management is open Monday through Friday from 8:00 a.m. to 4:30 p.m. The clinic is located at 850 West High St. It is on the northeast corner of Collette and High Streets, just north of St. Rita's Medical Center . Patients may use the parking lot in front of the building.

Disease Management:
St. Rita’s Disease Management Programs help patients take control of their conditions and live fuller lives. For the patient’s convenience, laboratory services on-site allow patients to have their blood draws without having to move to another location.
