You're not alone when facing joint replacement surgery, St. Rita's care managers are right beside you every step of the way-- before the surgery, through hospitalization and during the recovery process. Care managers use their expertise and commitment to guide patients and their families through the entire process. They work with patients and their families to develop a personalized discharge plan. They make sure the patient has all the needed medical equipment for home use. Any necessary referral to our social workers, home health care or other agencies will be made to assure appropriate and safe discharge from the hospital.
